GRANTEE REPORTING REQUIREMENTS

Annual grant reports are due within thirteen months of the date your grant was awarded. With multi-year grants, successive year’s funding is contingent upon satisfactory progress towards your goals. For all other grants, funding requests for the same project will not be considered until an annual grant report is received. The annual report should include:
  1. A description of the progress you have made toward achieving the goals and/or objectives of your project. This may include quantifiable attributes, e.g. the number of youth served, or unquantifiable attributes, e.g. the morale and attitude of students. Pictures and stories are always appreciated, but not required.

  2. A candid description of any set-backs, learning experiences or challenges met in the past year.

  3. A list of expenditures made from the granted funds.

  4. A listing of any change in relevant personnel, locations, etc.

  5. Objectives and/or goals for the future and how these will be accomplished.

  6. The following may be included if available but are not required:

    1. Annual report (required if requesting more funds)

    2. Financial statement (required if requesting more funds)

    3. Pictures, stories, artwork, newsletters, etc.
