Grant Guidelines

The Austin-Bailey Health and Wellness Foundation is committed to supporting programs that promote the physical and mental well being of the citizens of our community.


PREPARING YOUR GRANT APPLICATION

The trustees of the Austin-Bailey Health and Wellness Foundation want to give your proposal the consideration it deserves. Your grant application is the most important stage in the Foundation's evaluation of your proposal. For this reason, you should address each question asked, specifically and completely.

The first step in the application process is to complete a "Grant Request Cover Sheet." This form, listing some of the basic information regarding your organization, will assist our grant committee in determining if your request for a grant will be a match with our mission in the community. You will be asked to provide the following:

  • Objective(s) of the project
  • Description of your organization
  • Mission statement
  • Copy of current year's budget
  • Tax determination letter - 501©(3)
  • Prior year's audited financial statements
  • List of your board of directors

This information should give our review committee sufficient data to make a decision about the grant request. If not, we will follow up with a letter asking for clarification.

However, before attempting to complete any of the segments, please contact the Foundation's office by telephone at 330-580-2380. This could save valuable time in making your grant request. You will then receive specific instructions by mail to assist you in the process. Since incomplete proposals cannot be considered for funding, it is important that you follow the procedures outlined in our letters to you.


APPLICATION TIMELINES

Grants are awarded twice a year. Application deadlines are June 30 and December 30. The Foundation's board of trustees meets in March and September to review grant applications. Applicants will be notified no later than sixty days after the board of trustees' meeting. You may submit your proposal at any time during the calendar year.


ELIGIBILITY

Those seeking Foundation funding must:

  • Provide a current copy of IRS determination indicating their tax-exempt status.
  • Serve part or all of the four county area of Holmes, Stark, Tuscarawas, and Wayne in Ohio.
  • Provide a description of the organization and its activities.
  • State the need to be addressed.
  • Provide a brief description of the project or program and how it will address the health and wellness needs of the community.


INELIGIBLE REQUESTS

Austin-Bailey Health and Wellness Foundation does not generally grant funds for the following:

  • Annual appeals or membership drives.
  • Fund raising events.
  • Program advertising.
  • Lobbying activities.
  • Endowment funds.
  • Organizations not classified as tax exempt by the IRS.

Any proposal which has not been prepared according to the guidelines provided will be considered ineligible.

  • We emphasize healthcare issues of the uninsured and underinsured, the poor, children, single parents and aging.

  • We encourage and support the efforts of those needing financial assistance to further their education in the healthcare field.
